CLUB, PLAYER AND TEAM MANAGEMENT

Player Age Bracket Chart 2012 - 2013

Player Permission to Multi-Roster - use when a player wishes to play on two teams within one WYS affiliated club; or on two teams with two different WYS affiliated clubs. The state office and each club must approve this permission.

Interclub and/or Interstate Release Request - For WYS players who wish to 1) request permission be released from their current, primary CLUB for the purpose of joining another WYS affiliated club or, 2) to relocate to another STATE, during the current seasonal year.

This request is required when a player would like to another WYS affiliated club or a USYS affiliated state. It is the responsibility of the player and his/her parent or guardian to provide this form to the appropriate parties from WYS and USYS. The state office and each club must approve this request.

+2 Years Player Petition to Play Up - this must be submitted by the club president.

2012-2013 Additional Waiver and Release of Liability - for use with an approved +2 Years Petition to Play Up

Member Registration Form - includes a medical release and waiver of liability. This form is useful for gathing participant information for making or updating member profiles. If properly filled out this form can be used for a consent for medical treatment.

NEW WYS Yearly Re-affiliation and Board/Club Update Form - The term of membership for a Wyoming Youth Soccer affiliated club is one seasonal year (August 1 – July 31) and automatically renews on Aug 1. However, to continue to be in good standing with both WYS and US Youth Soccer this form and the affiliation fee must be submitted no later than November 1 of each seasonal year.
