an email to me then attach the saved travel notification

Once
I receive the email I will "sign it" and send it back to you
as a pdf document.

Print
that out and sign it.

If
you enter your roster on the 2nd page of the notification you do not
have to scan and email a roster.

The
BEST NEWS: once you have it saved to
your computer the next time you need to submit a Travel Notification
just open the saved file, make any changes and email.

team/game roster is already there and you can save some steps!

Rosters must consist of properly registered players who have player
cards.
When
to use a tournament/game roster
1
- When it has been determined that a team (by their age group) attending a
tournament must/should be comprised of players from more than one team
within a club.
2
- When, with the permission of each player’s coach, a tournament team
can be composed of players (of the same age group) from more than one
club.
A
tournament roster does not/cannot replace or be used for Wyoming League
competition.

no less than once every two years.

button on the form is the same as signing your name on a hard copy. You
therefore are agreeing to and authorizing a background check and declaring
that all information is true and correct.
