Official US Youth Soccer National Championship Team Roster: Please complete and submit the roster online here: FWR Roster Submission. The blank roster is available as a hard copy and can be emailed; handwritten rosters will not be accepted. this roster will be used for the Wyoming State Cup and Far West Regionals
A maximum of four team coaches/managers are allowed per team and on the sidelines.
Player’s name must match their player member card, which must match their birth certificate/proof-of-age document.
Nicknames, such as Bubba or Tiny cannot be used, but shortened names, such as Joe for Joseph or Sue for Susan can. Middle names are allowed (but not required) as long as they appear on the player's birth certificate.
WYS Player I.D.#
Date of birth, which must match proof of age document.
Jersey number and alternate jersey number (even if both numbers are the same).
Rosters will be approved by the WYS state registrar, stamped “official” and distributed at team check in.
IMPORTANT NOTICE: Players on each state champion team will be required to sign the official Official US Youth Soccer National Championship Team Roster immediately following their final game, before the awarding of their state championship. The coach or team representative must have team player cards with them at that time and be able to sign the roster. DO NOT let any of your players or coaches leave before this is done.
Laminated USYouth Soccer member passes, signed by Sandy Jenkin, state registrar, for the 2006/2007 season must be presented for each player and coach listed on the roster.
The pass will include:
The state association (Wyoming) and the region (RIV).
Player’s name, which must match roster and proof of age document.
Date of birth, which must match roster and proof of age document.
Team name, team number, team age group, team gender (B or G) and WYS player I.D. #.
The player’s signature, spelled exactly as shown on entered on the Wyoming State Cup/FWR roster and proof of age document.
A clear and current picture no larger that 1¼” x 1¼”.
A hole is to be punched through the upper left hand corner of the card (side with player info.), placed on an open lock ring (the rings that spread open), in roster order.
As determined by USYouth Soccer the following documents are acceptable as proof of age. None of these documents needs to be notarized. However, proof-of-age documents in a foreign language must have an English translation attached to the original document. Translations may be provided by anyone recognized as a translator by Wyoming Youth Soccer.
A copy of a U.S. birth certificate
A Uniformed Services Identification and Privilege Card (DD form 1173) issued by the uniformed services of the United States
Birth registration issued by an appropriate government agency
A passport
An alien registration card issued by the United States government
A certificate issued by the Immigration and Naturalization Service attesting to age
A certification of an American citizen born abroad issued by the appropriate government agency
Hospital, baptismal or religious certificates are not acceptable.

TREATMENT
A current copy of each player’s membership form or consent for medical treatment, properly signed, must be included.

Plan on at least 15 minutes for Credentials Check
Credentials will be reviewed following FWR guidelines. Any problems will be discussed with the team representative. If the team goes on to regionals any credentials problems must be fixed before submission to the state office.
Complete credentials are due in the state office no later than June 8.
Remove documents from the binder but leave in the plastic sleeves.
The copy of your official WYS team roster, which was submitted with your team registration, will be added to the last page.
In order to maintain the integrity of a team’s credentials no items will be removed or copied once the credentials have been received in the state office.
Team credentials
must be presented the following manner:
Each document is to be placed in a plastic sleeve in the following order and, for the Wyoming State Cup, placed in a 3-ring binder.
Coach and player passes are to be in roster order, on an open lock ring and attached to the second ring of the binder.
Front Binder Pocket: Will be used for game rosters. These will be given to the team representative at team check in.
First Page (right hand side): Keep this blank. Begin player credentials on the back of this page.
Proof of Age Document (left hand page): Place any translations in the same pocket, directly behind proof of age documents.
Highlight player’s name and birth date on birth certificate/ proof of age document.
Player Membership Form or Medical Consent (right hand page): Be sure the medical consent is signed by the parent/guardian.
Last Page: Leave blank.
Do not attempt to present team credentials that are not organized as required!

were uncontested and will be awarded the Wyoming state championship in their division. To receive the team award and to assure team credentials are in order for Far West Regionals a team representative must present to the state registrar the team's credentials, prepared as detailed above, during the weekend of the Wyoming State Cup.
